If you get that surgery done through the military, just like any other doctor visit you don’t have a choice who you see, they will pick the surgeon for you. Last time I checked they weren’t paying for lasik unless it was medically necessary.
Unless it’s changed in the last year, PRK is the choice. There are some military facilities that can do LASIK, but unless your eyes require it you will get PRK. As for who and where the surgery gets done it depends a lot on location. Being around McChord and Fort Lewis the only place in the area that does it is Bremerton Naval Hospital, so that’s where you go. As for expense, it’s a permissive TDY with convalescent leave at the end. You’re responsible for the costs of travel, lodging, and food. No per diem.
The military isn’t really big on giving you choices. They will tell YOU what you want to do.
